CHAMPVA Eligibility Requirements
Important Note
You must NOT qualify for TRICARE to be eligible for CHAMPVA. These programs cannot be used simultaneously.
You may be eligible if you are:
The spouse of a Veteran who has been rated permanently and totally disabled due to a service-connected condition
The surviving spouse of a Veteran who died from a service-connected disability
The surviving spouse of a Veteran who was rated permanently and totally disabled at the time of death
A survivor of a service member who died in the line of duty and do not qualify for TRICARE
Special Considerations
Age 65 and Older
If you are 65 or older, you must be enrolled in Medicare Part A and Part B to maintain CHAMPVA eligibility.
Remarriage
A surviving spouse who remarries after turning 55 retains CHAMPVA benefits.

How to Apply

Complete Application
Fill out VA Form 10-10d (Application for CHAMPVA Benefits) completely and accurately.
Submit Application
Mail your completed application and supporting documents to the CHAMPVA office for processing.
