How to claim it

  1. 1Apply for the placard first even if the plate is what you actually want. The agency will not process a physically disabled plate application without the placard behind it.
  2. 2Do not assume you need a physician. Six other practitioner types may sign the certification, including a chiropractor, a podiatrist and an optometrist — which matters if your treating clinician is not an MD.
  3. 3Check the mailing address on the form line by line, including apartment, suite or box numbers. The agency names address errors as the usual cause of a placard that never arrives.
  4. 4Give it twenty business days before chasing. After that, the Disability Parking Permit Desk will check the status.
  5. 5If your claim is service-connected, run this alongside the veteran routes rather than instead of them — the disabled veteran registration rate and the military plates are decided separately.
