How to claim it
- 1Apply for the placard first even if the plate is what you actually want. The agency will not process a physically disabled plate application without the placard behind it.
- 2Do not assume you need a physician. Six other practitioner types may sign the certification, including a chiropractor, a podiatrist and an optometrist — which matters if your treating clinician is not an MD.
- 3Check the mailing address on the form line by line, including apartment, suite or box numbers. The agency names address errors as the usual cause of a placard that never arrives.
