How to claim it

  1. 1Use the self-referral route if you have no case worker. The programme takes referrals through its own online form, through NJ Vet2Vet on 866-838-7654, and through NJ 211 — you do not need an agency to refer you.
  2. 2A community partner or an existing coordinated entry system can also refer you, which is usually faster if you are already working with a shelter or a service office.
  3. 3Ask for the specific service you need by name. The programme publishes street outreach, diversion, emergency shelter, housing navigation, rapid rehousing and regional coordination as distinct offerings.
  4. 4Contact the Office of Homelessness Prevention directly on 609-376-0811 or at OHP@dca.nj.gov if a referral stalls — the office runs the programme with the veterans department.
  5. 5Bring your DD214 if you have it. Nothing published requires a particular discharge for this route, but service verification will speed everything that follows.
  6. 6Ask your county Veterans Service Officer to work alongside the referral. Offices operate in all 21 counties and can pursue federal entitlements in parallel, which is usually what stabilises a housing situation for good.
