On July 1, 2009, Headstone and Marker Application Process was made effective by the Code of Federal Regulations. According to this law, a person who is applying for a headstone or marker must be the closest relative of the Deceased or the authorized representative of the Deceased. If not, then the application must include a written statement signed by the NOK or the authorized representative. There is no requirement for a notarized statement. Cemetery officials will order a headstone or marker based on the written information provided by the NOK.
