What if I have another disability not previously claimed?

If you have another disability that you have not claimed previously and if it is connected to your military service, you can file a new claim by completing VA Form 21-4138 along with a written request. You must submit your current medical records as well as any other information to prove your condition is service-connected. Please ensure you have included your Social Security number or your VA claim number on the completed form before mailing it to your VA Regional office.
