Presidential Memorial Certificate (PMC)

President John F. Kennedy instituted the Presidential Memorial Certificate, or “PMC”, program in 1962.  The PMC is an engraved certificate signed by the president to honor deceased veterans who were honorably discharged.

To apply for a PMC, download VA Form 40-0247, “Presidential Memorial Certificate Request Form”, available at http://www.va.gov/vaforms/va/pdf/VA40-0247.pdf.

  • You will need to provide a copy of the veteran’s DD 214 and death certificate when applying.
  • More than one certificate can be issued in order to accommodate additional survivors/loved ones.
