Did VA send you a letter asking you to furnish more details regarding your VA benefits claim or were you asked to send additional information to clarify an earlier claim? If yes, then you must send a completed VA form 21-4138 along with your statement to support your claim to the VA. This form can be downloaded at http://www.vba.va.gov/pubs/forms/VBA-21-4138-ARE.pdf . Please send the completed form to the VA Regional Office which has records of your claim. You can also visit the office in person.
