How are patient safety issues handled by VA?

In 1999, the VA’s National Center for Patient Safety (NCPS) was established. This was done with the goal of nurturing the tradition of safety through all the VHA (Veterans Health Administration). NCPS is dedicated solely to the prevention and reduction of accidental harm to patients nationwide. Patient safety officers at 21 regional headquarters and patient safety managers at 154 hospitals, governed by VA, participate in this program.
